High-intensity interval training helps older adults lose fat, retain muscle

In a sixโ€‘month study published in *The Journal of Gerontology*, researchers found that highโ€‘intensity interval training (HIIT) helped older adults lose body fat while preserving muscle mass. The trial followed 60 participants aged 65 to 80 who completed three HIIT sessions per week, each lasting 20 minutes, compared with a control group doing moderateโ€‘intensity steadyโ€‘state cardio. After 24 weeks, the HIIT group lost an average of 4.2โ€ฏ% of total body fat and maintained lean mass, whereas the moderateโ€‘intensity group lost only 1.5โ€ฏ% of fat but also shed 1.8โ€ฏ% of muscle.

Sarcopenia, the ageโ€‘related loss of muscle, is a major driver of frailty and falls in older adults. At the same time, excess abdominal fat increases the risk of cardiovascular disease and typeโ€ฏ2 diabetes. Conventional exercise guidelines have focused on moderateโ€‘intensity activities, which are safer but may not provide enough stimulus to counter muscle loss. Recent evidence suggests that brief, intense bouts of exercise can trigger metabolic pathways that preserve muscle while burning fat, but few trials have examined this effect in older populations.

The study randomized participants into two groups: the HIIT cohort performed 30โ€‘second bursts of cycling or walking at 85โ€‘95โ€ฏ% of their maximum heart rate, followed by 90โ€‘second recovery periods. The moderateโ€‘intensity group walked at 55โ€‘60โ€ฏ% of maximum heart rate for 30 minutes. Body composition was measured by dualโ€‘energy Xโ€‘ray absorptiometry, and waist circumference was recorded. The HIIT group saw a 6.8โ€ฏ% reduction in visceral fat and a 0.5โ€ฏcm shrinkage in waist circumference. Lean mass remained unchanged, with a mean difference of +0.2โ€ฏkg compared to a -1.5โ€ฏkg loss in the control group (pโ€ฏ<โ€ฏ0.01). Lead author Dr. Maria Sanchez noted, โ€œThese results demonstrate that older adults can safely perform highโ€‘intensity intervals and reap the dual benefits of fat loss and muscle preservation.โ€

If replicated in larger, more diverse cohorts, HIIT could be incorporated into community fitness programs for seniors, offering a timeโ€‘efficient way to improve metabolic health without compromising strength. Future research should explore the optimal frequency, intensity, and exercise modality for older adults, as well as longโ€‘term adherence and safety outcomes.
